Health status goes beyond medical boundries. Addressing the health status of individuals and communities requires attention to several factors.With the launching of Healthy People 2010, official and national attention was drawn to many of these factors, not only eliminating health disparities, but the factor of access to health care. Eleven years ago, through a community academic partnership in Dayton, Ohio, a model of improved access to health care was developed. The community-based model is grounded in health promotion and disease prevention, community empowerment, and outreach to vulnerable populations. The presentation will include development of the community-based model, outcomes of several programs addressing health access, and lessons learned.
