HIV prevention case management with HIV positive persons: Results from Wisconsin

Background: The jurisdiction’s prevention case management (PCM) program serves HIV-positive clients who engage in sexual and drug risk behaviors that may expose uninfected partners to HIV. PCM uses an intensive, culturally competent, client-centered intervention based on the stages of change model to help clients reduce their risk behaviors. Development of evaluation methods and tools has been integral to the evolution of the service delivery model. Methods: Clients complete a Behavioral Risk Assessment Tool (BRAT) by interview at baseline and at 3-month intervals. The tool collects data on sexual and drug risk behaviors and co-factors for HIV infection and transmission. Using the Client Functioning Assessment Tool (CFAT), staff assesses changes in clients’ life circumstances in areas that bear upon HIV risk behaviors. Data regarding session dose and content are also collected. The program has served 250 clients; 128 have completed baseline BRATs, 62 have completed follow-up BRATs, and 129 have completed baseline CFATs. Results: For the 62 HIV-positive clients with follow-up risk assessments, declines are shown on sexual risk behaviors. These include percent reporting any recent vaginal or anal sex (61% to 53%), percent with recent unprotected vaginal or anal sex with partner of negative or unknown HIV status (32% to 26%), and sex with an injection drug user (17% to 2%, p<0.01). Conclusions: The jurisdiction’s PCM for HIV-positive clients appears to be effective in reducing some sexual and drug risk behaviors. Discussion of programmatic and evaluation issues provide a context for the findings.
