DESCRIPTION /GOALS OF THE PRESENTATION:

This presentation will describe and discuss the use of Community Care Coordination (community health work), the Settlement House approach and a formal helping system combined to create an effective strategy to address infant mortality in the African-American community. Community Health Work is locally recognized as Community Care Coordination. The presentation will be co-presented by a Community Care Coordinator (CHW).
