Preventing kidney disease: A multi-pronged approach

End-stage renal disease (ESRD) is increasing such that it now represents a significant public health burden. As a result, the National Kidney Disease Education Program (NKDEP), an initiative of the National Institute of Diabetes and Digestive and Kidney Diseases, has been launched to reduce the incidence and economic burden of chronic kidney disease (CKD) and ESRD. This paper describes the innovative multi-pronged approach of NKDEP. To institute systemic-level change a number of working groups are undertaking such activities as: a) distilling guidelines and evidence-based efforts of multiple organizations into simple diagnostic and treatment targets; b).standardizing and improving laboratory measurements; c) encouraging regular reporting of glomerular filtration rate—a superior measure of kidney function; and c) developing CKD quality indicators for Medicare beneficiaries hospitalized for CVD in partnership with the Centers for Medicare and Medicaid Services. A cornerstone of NKDEP’s efforts is its “You Have the Power to Prevent Kidney Disease” campaign pilot-tested in four sites. Adhering to the principles of Social Marketing, the campaign targets African-American adults—a population at especially high risk—and their health care providers using a variety of audience-tested materials and employing both community-based outreach and mass media tactics. Family members of ESRD patients are at increased CKD risk; thus, NKDEP also has developed materials targeting ESRD patients that both encourage and facilitate discussion of CKD with their family members. Findings and lessons learned based on an extensive process and outcome evaluation will be discussed.
