Public health nurses competences in Japan: A five years longitudinal study

Purpose: In Japan, the environment of surrounding public health nurses (PHNs) has changed remarkably. This report is about the competence of public health nurse in Japan. Method: PHNs working for local government were surveyed using a self- administrated questionnaire sent by mail in 1996 and 2001. In the initial randomized stratified study, 398 questionnaires were sent and 277(70%) replies were received. In 2001, questionnaires were sent to all 1860 PHNs eligible an of 1131(61%) replies were received. Their competence was measured by a scale consisting 15 items established by the Japanese Nursing Association. The competence structure was analyzed using factor analyses, and self-evaluation was analyzed using the t-test or one-way ANOVA. Result: In both studies, the competence consisted of 2 factors, "interpersonal care competence (ICC)" and "community care competence (CCC)", but their contents were different. There were significantly differences in the 15 items of the competence among 4 career groups divided by years of experience. Members of group1 (1-5y PHN) ranked 5 items of the ICC lower than in the first study. In group2(6-10y), 5 items were ranked higher, and 3 lower than in the first study. Group3(11-20y) had higher self-evaluations for 5 items of the ICC, and in group4 (>20y), 1 item of the ICC received a higher self-evaluation.
