How well do women in Rockland County New York adhere to recommended mammography screening?

In the U.S., breast cancer is the second leading cause of cancer death among women. Breast cancer deaths can be reduced through early detection using mammography. Guidelines recommend mammograms every 1-2 years for women aged 40 years and over. The objectives of this analysis were to: 1) estimate the prevalence of guideline adherence in Rockland County, NY; and 2) explore how the prevalence differs across demographic subgroups. In 2002, a telephone survey was administered to a random sample (n=1,659) of women (aged 40 years and over), drawn from all residents of Rockland County. The demographic distribution of the sample was similar to the expected distribution, based on census figures. To address the objectives, percentages were calculated; and chi-square tests were conducted. P-values <0.05 were considered significant. Overall, 91.7% of women reported receiving a mammogram within the past 2 years. Older women (50+ years) had significantly higher adherence proportions than their younger counterparts (93.0% vs. 89.1%, respectively). Higher adherence was also associated with a college education (vs. no college education; 92.7% vs. 89.7%), Jewish heritage (vs. Non-Jewish heritage; 94.2% vs. 90.7%), and health insurance coverage (vs. no coverage; 92.5% vs. 62.1%). There were no significant differences by race/ethnicity, marital status, or employment status. In Rockland County, there was high overall adherence to recommended mammography screening. However, there is one demographic subgroup (uninsured women) that should receive first attention regarding interventions designed to increase the use of breast cancer screening services.
