C. Meade Grigg, MA, Christine M. Abarca, MPH, CHES, Debbie Reich, MA, and Jo Ann Steele, MPH. Office of Planning, Evaluation and Data Analysis, Florida Department of Health, 4052 Bald Cypress Way, Bin A05, Tallahassee, FL 32399-1706, 850-245-4444 x 2071, Christine_Abarca@doh.state.fl.us

Florida Department of Health’s statewide community health assessment initiative sets as its mission the enhancement of capabilities of local public health systems to conduct community health improvement planning and the application of results for improved poppopulation health outcomes and public health policy formulation. Florida's initiative builds infrastructure and capacity through policy formulation for systematic performance of comprehensive community health assessments; establishment of performance standards; development and access to Florida’s web-based data system (Community Health Assessment Resource Tool Set, CHARTS) and tools designed to advance the technical skills of local public health system partners; public health workforce skills development through multi-modal training opportunities and technical assistance; and evidence-based evaluation and continuous quality improvement of the statewide initiative as well as enhancement of local processes. Florida's initiative utilizes the National Association of County and City Health Officials’ (NACCHO) MAPP planning model as the framework for the development of tools and processes. Implementation plans are designed and strategies identified for each county health department based on current capabilities while meeting or exceeding statewide performance standards. Through this initiative, county health departments and the communities they serve have access to health-related data via CHARTS, web-based and regional training for skills enhancement, reference materials including field guides and web-based templates and tools, marketing and public information materials for raising awareness of the benefits and impact of assessment, performance benchmarks and quality improvement standards for county health departments, and quantitative and qualitative evaluation measures and reports on both processes and outcomes.
