NYC Health and Hospitals Corporation (HHC) is a public benefit corporation (11 acute care, 4 long term care, 6 diagnostic and treatment centers and a health and home Care agency) that provides health care services to a largely disadvantaged (multicultural and multiethnic) population, with variety of chronic diseases and little attention to preventative care. Scientific- and evidence- based studies have shown that Quality Improvement (QI) approaches has made significant impact on health promotion and disease prevention.
The HHC Quality Assurance Committee of the Board of Directors identified several health QI Indicators to promote health and prevent chronic diseases. These Indicators were implemented with the goal to standardize Quality Improvement measurement methodologies and implement corporate-wide best practice guidelines. The following are some of the indicators that were implemented:
smoking cessation colon cancer screening sleep positioning of infants asthma control cholesterol/BP control lipid level domestic violence screening follow-up of mental health pediatric dental visits
Implementation of these indicators resulted in an increase of 80% of HHC patients being screened using the above indicators and appropriate treatments were implemented.
Learning objectives: 1. Identify QI indicators that will impact on health promotion and disease prevention of the city’s minority, indigent and disadvantaged population. 2. Describe QI monitoring processes to identify and implement Best Practice Guidelines.
