Community partnerships to provide preventive care to children in underserved areas

In a time of decreasing resources, public/private partnerships may serve as models for community-based health initiatives. The Community Access to Child Health (CATCH) Program is an American Academy of Pediatrics (AAP) initiative, and the Healthy Tomorrows Partnership for Children Program (HTPCP), is a collaborative partnership between the AAP and the federal Maternal and Child Health Bureau (MCHB). These national community-based health programs encourage health professional involvement in community-based initiatives through training, technical assistance (TA), networking and funding opportunities. These programs promote a community partnership approach to ensure that underserved children and their families have access to care. The CATCH and HTPCP programs help teams of multi-disciplinary health care professionals create comprehensive community-based health programs to address the needs of the target populations. These programs address a variety of health and environmental issues and have been successful in encouraging awareness of resources in the community as well as working towards reducing the duplication of services. Community commitment and the identification of key players at the onset is critical to the successful planning, implementation, and sustainability of a program. This presentation will illustrate successful partnerships and discuss strategies, including TA (eg. identification of funding resources, coalition building, and program evaluation) to encourage health care professional involvement in preventive and primary care services at the community level.
