Population-based public health: Connecting the pieces

If the public health system is to achieve its goal, it is critical that everyone --ranging from the top manager to the front-line practitioner -- is able to articulate what they do, why they do it, and how it contributes to improving population health. This requires public health practitioners to be able to integrate the various components that guide and direct public health practice. This session will define population, population health, and population-based practice. It will describe the components that guide population-based public health practice: the principles of public health, the core functions, the ten essential services, national, state, and local goals, public health programs and public health interventions. It will present a model that articulates the relationships between these components and how each of the components contributes to improving population health. The model depicts “why” public health practitioners do what they do (the public health principles), “what” public health practitioners do (the core functions, ten essential services, and national, state and local public health goals) and “how” public health practitioners accomplish their work (public health programs and the public health interventions). This session will illustrate these relationships with examples of current and historical public health population-based practice. (This model is currently being utilized in a distance education course at the University Of Minnesota School of Public Health.)
