Community Health Workers design, develop and implement a home-visiting program - a NYC story

Health Plus is a pre-paid health services plan serving all culturally diverse communities of New York City. Since 1995, Health Plus has operated a telephonic community health outreach program to improve member's access to quality health care services. This outreach effort has recently expanded to include home visiting interventions. With guidance and support from management, CHWs themselves designed and implemented all aspects of this new home visiting program. This presentation by community health workers will describe that journey, share the challenges and celebrate the successes of this program.
