Coordinated Care for Children with Special Health Care Needs: State and Local Efforts
Healthy People 2010 calls for increasing the proportion of States that have service systems for children with special health care needs (CSHCN); and the national agenda for CSHCN focus on improving access to a medical home; promoting meaningful family participation in medical decision making and policy development; implementing early and continuous screening; making systems easy to use; increasing availability of public and private health insurance, and facilitating transition of youth to adulthood. This poster session highlights efforts at state and local level to implement community-based systems and address one or more of the national goals for CSHCN and their families
Learning Objectives: Increase awareness of the efforts at the state and local levels to address one or more of the six goals of the national agenda for CSHCN and their families.
