PACE is managed care program, providing acute and long term care services to frail older persons. Its objective is to maintain participants’ independence in a community setting for as long as possible. The program is innovative in its approach to care and service provision, emphasizing the care team and services in a day care setting. It offers a promising alternative to nursing home care. In this session we will present 6 talks. The first will describe PACE, its unique features, and its evolution from a demonstration to a Medicare/Medicaid provider. We will then present 4 studies that examine the inner workings of the program: 1) The relationships between program structure, processes and participants’ risk-adjusted health outcomes; 2) The relationship between the performance of the care team and participants risk-adjusted health outcomes; 3) Disenrollment from PACE – who is leaving and why? And 4) The potential growth for PACE – barriers and facilitators. We will then conclude the session with remarks from the National PACE Association reflecting on the studies presented and discussing the future for PACE and other similar initiatives. | |||
