Medication management pilot project: A clinical service for community-dwelling elders in Nevada

The Sanford Center for Aging of the University of Nevada, Reno, with funding from the Nevada Division for Aging Services has implemented a Medication Management Pilot Project (MMPP) for seniors 60 years and older throughout the state, who are currently taking eight or more prescription medications. Through this program, the clinical services of geriatric certified pharmacists have been provided to over 250 community-dwelling seniors. Pharmacists have identified drug interactions, drug duplications, adverse reactions, side effects, and inappropriate prescribing practices. Recommendations responding to these issues have been provided to seniors and in some cases to their case manager or social worker by the pharmacists. Empirical data on the incidence of drug-related medication management issues, the effects of a geriatric certified pharmacist's review, and the subsequent health-related outcomes has been gathered for participants from June 2004 to the present. Participants who attend this session will receive an analysis of the results of the MMPP, information about the type of clinical services geriatric certified pharmacists can provide for older adults, the value these services have in relation to gerontological health, and the need for this type of service for community-dwelling seniors. Learning Objectives: 1. To articulate the need for the reimbursement of clinical medication review services provided by geriatric certified pharmacists. 2. The ability to develop a program model for the provision of medication review services in other communites based on the design of the MMPP. 3. Apply the value of this practice to the creation of appropriate health policy and legislature.
