Customized training for health care providers to integrate smoking cessation intervention into prenatal settings

Mom's Quit Connection (MQC), a smoking cessation program funded through the New Jersey Department of Health and Senior Services, offers counseling, education, and outreach to pregnant and parenting women in New Jersey. MQC receives client referrals from prenatal providers via postcard, telephone and fax. Historically, approximately 30% of those patients referred actually enter treatment. Providers rarely follow up with patients beyond the initial referral. To capitalize on the patient-prenatal provider relationship and utilize opportunities for ongoing intervention, MQC initiated on site training to implement the American College of Obstetrics and Gynecology's (ACOG) 5 A's brief intervention model into providers' offices. The 5 A's; ASK, ADVISE, ASSESS, ASSIST, and ARRANGE recommends a 5-15 minute intervention at each prenatal visit to assess readiness to quit and provide quitting strategies and additional resources. MQC adapted this 5A's approach to create a training model unique for providers. Model components include :ASK providers how they are presently addressing maternal smoking in the office; ADVISE providers that free onsite training is available to all NJ practices; ASSESS the current patient flow during routine prenatal visits; ASSIST offices in incorporating the (ACOG) 5A's into their practice; ARRANGE for each office to have access to MQC's more extensive case management services. Statewide, MQC offers individualized telephone counseling, stage based, pregnancy specific materials and ongoing follow up to any referred patients. Early results demonstrate that offices trained by MQC are more invested in their patient's quit attempt and more frequently utilize MQC resources to facilitate positive results.
