Purpose: In fall 2004, the Pasadena Public Health Department launched the MAP Campaign, a multi-faceted community-wide strategic planning effort to advance community health. Modeled after the CDC/NACCHO Mobilizing for Action through Planning and Partnerships (MAPP) process, the MAP Campaign mobilized community wisdom, resources and expertise to identify and tackle the City's most challenging health concerns. Methods: A year-long comprehensive and inclusive planning process yielded a strategic plan that specifies community priorities and collaborative strategies for improving our local public health system and health outcomes. Faith institutions, non-profits, healthcare providers, residents, businesses and government officials have been central players in: 1) establishing a shared vision for community health; 2) examining our local health status; 3) measuring local public health system capacity; and 4) reviewing and understanding local, regional and national challenges and opportunities that influence how we plan for, improve and sustain community health. In addition to discussing the approach, challenges and achievements of the MAP strategic planning process, this presentation will highlight creative mechanisms used by the Pasadena Public Health Department to develop, execute and propel this City-wide initiative despite scarce resources. Results/Conclusion: The MAP Campaign has engaged over 1500 participants in the creation of the MAP strategic plan. Nearly 20 agency and community leaders comprised the MAP Steering Committee and collaborated fully on convening three large-scale information sharing events, distilling assessment results, setting priorities and leading action steps to improve health and influence public health policy and resource allocation. The MAP strategic plan is slated for release fall 2005.

Learning Objectives: At the conclusion of the session, participants will be able to
