Empowering district policy makers to promote contraceptive security in a decentralized district: Experience from Indonesia

Family planning program in Indonesia was decentralized in January 2004 to district government following the decentralization of the other sectors since 2001. Local managers, because they are closer to clients, should be able to make better resource allocation decisions than central-level officials. Unfortunately the resources are limited in most of the districts. Unless the stakeholders for family planning program at district level can collectively agree to prioritize the supply need, contraceptive security - the ability of every person to choose, obtain, and use good quality contraceptives when needed - is at risk. A district planning tool for contraceptive security has been developed, modeled after the performance improvement framework and the SPARHCS (Strategic Pathways to Reproductive Health Commodity Security) tool adapted for district level use. Implementation of the tool in a district in Indonesia has contributed to building a strong team spirit and empowered the multidiscipline stakeholders of family planning program. A series of decisions have been made by them for public sector as well as private sector. Among others, decision made through a complex process of decisions, involving the local government and parliament, resulted in the allocation of government budget resources for procurement of contraceptives for the first time. Decision to deregulate the procedure for getting the permit to open new pharmacy, has resulted in establishment of 10 new pharmacies in the area where there was no access to pharmacy service before.
