The presentation will focus on the ongoing experience in evidence-based service delivery inititiative at St. John's Health System, located in Springfield, MO. St. John's Health System is a fully integrated system affiliated with Sisters of Mercy Health System, St. Louis. Combining six hospitals, 435 physicians and a health plans division, St. John's Health System ranks fourteenth in the 2005 most integrated systems evaluation. St. John's Physicians and Clinics is also one of ten CMS awardees for a pay-for-performance Medicare fee-for-service demonstration.

St. John's Health System combines a health plans data warehouse with sophisticated analytical capability to identify utilization and demand management opportunities and stratify patients for disease management interventions. Supplemented by clinical data feeds and supported by a disease registry, St John's Health System has four years of demonstrated success in improving quality and reducing medical cost trend for St John's Health System, local employers and other Sisters of Mercy regional health systems.
