Reproductive health indicators remain poor in many countries despite decades of innovative, successful small-scale projects. The demonstration effect alone has been insufficient to achieve the large-scale adoption of these innovations necessary to effect better outcomes. Management Systems International (MSI) developed a framework for "scaling up" successful small-scale projects, field tested it with several MacArthur Foundation-funded reproductive health projects in Nigeria and Mexico, and revised it based on lessons learned. The resulting framework is an innovative application to the reproductive health field of development management tools that MSI has used in its consulting practice,. This paper presents the results: a three stage, ten step process for scaling up, with specific tools for implementing each step. Stage 1: Creating a Vision and Broad Strategy: what is being taken to scale, by who, how and where i. Clarifying the Model ii. Choosing the Method and Type of Scaling-up: replication, expansion or collaboration iii. Determining Organizational Roles iv. Establishing Dimension(s) Stage 2: Establishing the Preconditions for an Effective Scaling-up Process i. Legitimation ii. Constituency Building iii. Realigning and Mobilizing Resources Stage 3: Implementing the Scaling-up Process i. Modifying Organizational Structures ii. Coordinating Action iii. Performance Monitoring Also included in the paper are: · the key capacities and resources needed in an organization engaged in scaling up; · a discussion of the alternatives of having an organization scale up its own innovation versus the role that a third-party can play in filling those roles; · main lessons and specific illustrations from Nigeria and Mexico.
