Background: The number of deaths among persons with HIV/AIDS has been decreasing in the post-highly active antiretroviral therapy (HAART) era. This study is to investigate whether other causes of death unrelated to HIV/AIDS have significantly impacted persons with HIV/AIDS. Methods: A total of 14,920 deaths among persons reported with AIDS from 1990 to 2002 were identified through record linkage between the Los Angeles County's AIDS registry and Vital Records. Multivariate logistic regression model was used to examine the risk for the causes of death unrelated to HIV/AIDS. Results: The proportion of non-HIV/AIDS related deaths among persons with AIDS increased from 12% in 1990 to 22% in 2002. The proportion of deaths due to influenza & pneumonia and other infectious disease decreased significantly from 25% in 1990 to 8% in 2002, while the proportion of deaths due to cardiovascular diseases increased from 8% in 1990 to 24% in 2002. An increase was also observed in proportion of deaths due to chronic liver disease and cirrhosis (2% in 1990 and 8% in 2002), and unintentional injuries (2% in 1990 and 8% in 2002). Among persons with AIDS, whites were 50% more likely die of cardiovascular diseases when compared to other racial/ethnic groups (adjusted OR: 1.5, 95% CI: 1.2-2.0). Conclusions: Cardiovascular diseases played an increasing role for deaths among persons with AIDS. Treatments and prevention programs may be targeted to prevent these chronic illnesses among person with HIV/AIDS.
