Sergio Matos, Manager, Community Health Outreach, Health Plus, PHSP, Inc., 241 - 37th Street, Brooklyn, NY 11220, 718-491-7575, smatos@healthplus-ny.org, Anne Willaert, Minnesota State Colleges and Universities, Healthcare Education Industry Partnership, 102 Wiecking Center, Mankato, MN 56001, and Carl H. Rush, MRP, Chommunity Health Worker Institute, New Jersey AHEC Program, 42 East Laurel Road, Suite 3200, Stratford, WA 08084.

The community health worker model is an effective means of helping healthcare institutions provide socially and culturally appropriate services and helping communities access those services thereby bridging gaps between individuals and health/social systems. Community health workers are trusted members of the communities they serve and therefore have unique ability to improve the use of primary care; enrollment in public health programs; adherence to treatment strategies and health outcomes in general. Their ability to create this link and improve health outcomes is advantageous to healthcare institutions. This presentation makes a business case for the application of the community health worker model in health and social systems.
