This interactive session will explore successful strategies for developing evidence-based policy and practice that leads to reducing health disparities. Panelists will address how to define measures for determining success of evidence-based practices for diverse communities including quality of life. It will also explore the barriers to policy development and practice such as ensuring the inclusion of diverse communities. Panelists will highlight the ethical framework and cross cutting interventions that have been developed to address this issue across all communities. This session will also provide a funder’s perspective on ensuring the sustainability of these evidence-based policies and practices.
