Review of the literature on the cost-effectiveness of tobacco use interventions

Objective: Review the literature on the cost effectiveness of tobacco use interventions to summarize major results, identify gaps in the literature, and suggest future research. Methods: Systematic review of 40 peer-reviewed articles on cost-effectiveness of various tobacco use interventions, including cessation, tax and price increases, media campaigns, quit lines, provider reminders, youth access prevention, and community interventions. Results: Across the full spectrum of tobacco use interventions, the median cost per life year saved is $1,400 and the median cost per quality-adjusted life year saved is $4,400, which compares favorably with other public health interventions. Cessation interventions, including nicotine replacement therapy, are the most heavily studied group of tobacco use interventions. Much less work has been done establishing the cost-effectiveness of other interventions. Conclusions: Tobacco use interventions of all kinds are some of the most cost-effective public health interventions. Tobacco control professionals can use these beneficial cost-effectiveness results to argue for sustaining tobacco control programs. More work needs to be done to establish the cost-effectiveness of common interventions, such as media campaigns and clean indoor air laws.
