Prenatal screening for infectious diseases: An analysis of disparities and adherence to policy in California

Background: Perinatally-acquired infectious diseases are a significant cause of preventable infant mortality. This study aims to use existing epidemiological data to evaluate adherence to policy requirements regarding prenatal infectious disease screening as well as to determine factors accounting for variations in testing.

Methods: The study consisted of an analysis of an existing database of 1835 live births in between 1999-2003 from 7 California counties. Labor and delivery and prenatal care records were abstracted for demographic, prenatal and disease testing data. Variables associated with screening for GBS, HIV, syphilis, rubella and hepatitis B were evaluated by univariate and multivariate analyses.

Results: Syphilis (98.2%), rubella (97.3%) and hepatitis B (96.5%) had higher rates of testing than HIV (57.2%) and GBS (52.0%). Younger women, women with fewer births and those with MediCal or private insurance versus no insurance were significantly more likely to receive testing. Lack of prenatal care and a primary language other than English or Spanish were barriers to testing.

Conclusion: Adherence to policies for prenatal testing is variable by both disease and maternal factors, but is necessary to improve perinatal disease prevention. Additionally, adequate and high-quality prenatal care may increase opportunities to provide infectious diseases testing during the prenatal period.
