Evaluating a national family planning quality improvement program in Egypt

Objective: This study evaluates whether the family planning facilities in Egypt that were certified as Gold Star during 1995–2000 have sustained higher quality services than non-Gold Star facilities four years after the conclusion of the program.

Design: The Gold Star and non-Gold Star facilities are compared on several indicators of quality of family planning services and provider-client interactions using both descriptive and multivariate methods. Data are from the 2004 Egypt Service Provision Assessment Survey.

Setting: Health care facilities providing family planning services in Egypt.

Participants: A nationally representative sample of 637 facilities providing family planning services and 1,930 observed provider-client consultations.

Intervention: The Egyptian government conducted a national Gold Star program during 1995–2000 to improve the quality and usage of family planning and certified the facilities that met certain quality criteria.

Main Outcomes Measures: Indicators of availability of infrastructure, services, supplies, and provider-adherence to standard practices in counseling and examination.

Results: The Gold Star facilities have significantly better availability of family planning methods and counseling and examination services than non-Gold Star facilities, independently of the type, size, and geographical location of the facility. The providers in the Gold Star facilities are significantly more likely to adhere to higher quality practices in counseling and examination.

Conclusions: The facilities that were certified as Gold Star continue to provide higher quality family planning services than non-Gold Star facilities.
