Improving access to postabortion care (PAC) services in Mozambique

Background: With funding from the Hewlett Foundation, Pathfinder International conducted a qualitative study in Xai Xai City to determine popular perceptions and knowledge regarding abortion and related care. The study was conducted between June-July 2005 within the context of Geracao Biz, an adolescent SRH program implemented by UNFPA, Pathfinder, and the Government of Mozambique.

Methods: Methodology included: 1) an assessment of abortion practices and PAC through interviews with health professionals and PAC clients, and visits to health facilities offering PAC; 2) focus group discussions with young women and men (aged 15-20) and community leaders to explore practices, beliefs, and values related to abortion and PAC; and 3) dialectical analysis of data.

Results: Community knowledge and beliefs about the prevention of unwanted pregnancy and unsafe abortion need to be clarified and access to quality PAC must be improved. Negative community attitudes toward discussing sex and SRH with youth hinders healthy behaviors and access to quality PAC. When faced with an unwanted pregnancy, youth often consult traditional healers, believing they are more apt to offer privacy, confidentiality, and a non-judgmental attitude. Community mobilization and an effective communication strategy is needed to prevent unwanted pregnancy and unsafe abortion, and promote access to quality services.

As a result of this study, Pathfinder conducted training in PAC and improved the quality of PAC services at Xai-Xai Provincial Hospital emphasizing privacy, confidentiality, provider attitudes, and counseling. Pathfinder will also assist the MOH in developing guidelines for PAC and postabortion counseling and train additional providers in PAC.
