An inner city school based health center partners with the school administration to achieve an immunization rate of 98% among the entire school population

Immunizing children that live urban, poverty stricken areas is a challenge across the country. Michigan Medicaid HMO's struggle to report HEDIS immunization rates above 90%. In some areas the rates are in the 70% range. Michigan has developed a computerized registry in which clinicians are required to enter immunization data at the time of administration. St. John Health operates twelve school based health centers in the Detroit area to achieve a corporate mission objective of “health care that leaves no one behind”.

One school based health center has developed a program in which a staff member looks up the immunization status of each child in the school in the immunization registry. The names of children that are incomplete are provided to the school administration and the children are sent to the health center for immunization administration after appropriate permissions have been obtained. This program resulted in an immunization rate of 98% at this school.
