Cost and functional use patterns of rural Medicare participants: Research and policy implications

Session Abstract-Introduction to Symposium. Gerald M. Eggert, PHD, Moderator. We recently concluded a randomized trial of Consumer-directed self care strategies with a high risk, high cost Medicare population (N=1,604) who lived in parts of NY, WV, and OH. Consumers who lived in rural areas and received a primary care affiliated nurse intervention maintained their functional (ADL) status better than those subjects who lived in urban areas. Dr. Brenda Wamsley presents change scores in ADL for those consumers who completed the 22 month measurement period by rural-urban residence. Dr. Daniel Meng presents ADL change scores and Medicare and private cost for those who had heart conditions. Dr. Bruce Friedman presents ADL change scores and cost for those consumers who were cognitively impaired. Dianne Liebel, BSN, ABD, concludes with a description of what the intervention nurses actually did and discusses what she felt they did to achieve the positive ADL outcomes. Each presenter will also offer their explanation as to why the intervention seemed to be more effective in rural areas than in urban areas. Future research and policy implications for rural health will be discussed by Dr. Joan Van Nostrand, Research Director, Office of Rural Health Policy, HRSA/DHHS. Questions will be sought from the audience.
