Planning diabetes intervention efforts with geographical information systems

Data from a community survey were analyzed geographically to help facilitate local diabetes prevention efforts. Data were available from the 2003 Speak to Your Health! Community Survey, a community-based survey designed and implemented by university and community partners in the Prevention Research Center of Michigan to investigate health and social issues in Genesee County, Michigan. We used survey data matching U.S. DHHS criteria to calculate and geographically map diabetes risk scores and also mapped diabetes screening rates with 2003 survey data. Maps indicated that those areas where the estimated risk of diabetes was the highest had only moderate rates of diabetes screening relative to other areas. We presented these results and maps to two community groups focusing on diabetes prevention intervention activities and identified the areas where diabetes risk was high and screening rates were low. We replicated our analyses from the 2005 Speak to Your Health! Community Survey and also mapped the proportion of respondents who had been diagnosed with diabetes. The 2005 maps indicated that the proportion of respondents at high risk for diabetes increased across nearly all ZIP Codes. Diabetes diagnoses matched the pattern of diabetes risk estimates. The percent of respondents who were at high risk for diabetes and had not yet been screened was considerably lower in the areas that had been identified for intervention efforts by local diabetes prevention groups than in surrounding areas.
