Use of alcohol treatment services and unmet need among men in the general population: The intersection with intimate partner violence and race/ethnicity

Aims: To examine the relationship between IPV perpetration and 1) utilization of any alcohol treatment, and 2) unmet need for alcohol treatment (those with alcohol abuse or dependence that did not receive treatment in a specialty facility) in the past year among Black, Hispanic, and non-Hispanic White men in the general population. Methods: The 2002 National Survey on Drug Use and Health was utilized and included Black, Hispanic, and White male cohabiting respondents 18-49 years of age who were drinkers in the past year. Logistic regression was used to calculate adjusted odds ratios (AOR) and 95% confidence intervals (CI). Results: Approximately 2.5% of perpetrators and 0.6% of nonperpetrators received any alcohol treatment; 34% and 15%, respectively, needed but did not receive treatment. Perpetrators were more likely to need but not receive treatment (AOR 1.63; CI 1.04, 2.54); younger age at first drink, number of drinks per week, illicit drug use, drug abuse/dependence, and serious mental illness were independent risk factors. In race/ethnicity-specific models, Black perpetrators and to a lesser extent Hispanic perpetrators, but not White perpetrators, were more likely than their nonperpetrator counterparts to have unmet need for treatment. Perpetrators were no more likely than nonperpetrators, however, to receive any alcohol treatment, although age at first drink, illicit drug use, and drug abuse/dependence were independent risk factors. Conclusions: Alcohol and other substance abuse and mental health needs should be assessed among men identified as engaging in IPV. It may be beneficial to screen for IPV among alcohol treatment clients.
