Service integration to reduce the burden of asthma: Is it effective?

The rising prevalence of asthma and asthma-related morbidity and mortality has lead communities to search for ways to effectively improve disease management and coordinate asthma-related community resources. In order to improve access to and delivery of asthma services within the Medicaid Managed Care (MMCO) population in Philadelphia, the Philadelphia Allies Against Asthma (PAAA) Coalition developed community partnerships and adopted a comprehensive approach to integrate, improve access to and expand services to families with children with asthma. A corner stone of the service integration is the Child Asthma Link Line, a telephonic bilingual care coordination for families with children with asthma. Families are followed for a period of four months to assure that they receive the care that they desire and need.

This paper describes the outcome findings of the PAAA evaluation. The data were gathered through three sources: MMCOs, parents who contacted the Link Line, and parents and students at 16 study schools (eight intervention and eight intervention). The findings suggest that an intervention based on community partnerships and integration of asthma-related services is related to positive outcomes for children and can reduce the burden of asthma in a large urban setting.
