“Women Take Pride” is an evidence-based self-management intervention for older women with heart disease that has been shown to have positive effects on physical functioning, ambulation, symptom experiences and body weight among women recruited from clinical settings. It is being implemented in community settings by an interdisciplinary partnership consisting of a senior service agency, a health care organization, an area agency on aging, a health insurance company, and an academic institution. To effectively translate the program to community settings, it was necessary to modify and adapt the focus of the intervention to appeal to a broader target population, while maintaining fidelity to the intervention curriculum. Preliminary 4-month outcome data suggest that outcomes in this demonstration compare favorably with outcomes from the original research trial for physical functioning, body weight and symptom experience. This presentation will share detailed outcome findings to date. The presentation will also focus on the roles of each partner in implementing this program in community settings, the strategies used to maintain and assess fidelity, and the successes, challenges and lessons learned in the translation process.
