Disparities in stroke and prostate cancer mortality rates, based on race/ethnicity, are evident among men in Pennsylvania. African American (AA) men in Pennsylvania, and particularly in Philadelphia, are more likely to die of a stroke or prostate cancer than are men from different racial/ethnic backgrounds. The Stroke Hypertension and Prostate Education Intervention Team (SHAPE-IT) project is a 2.5-year community-based intervention that fosters partnerships between community-based organizations, community members and health care providers to design and implement strategies that ultimately can reduce disease and death from prostate cancer and stroke among AA men age 35 and older in North & West Philadelphia. Using Community-Based Participatory Research (CBPR) Methodology, SHAPE-IT plans to reach at least 25% (6,750) of the 27,000 AA men residing in the target areas. This project provides substantial opportunities for community planning, establishing partnerships, and enhancing cultural relevance of activities by using a shared responsibility model of community health planning. Project activities included: 1) the development and implementation of a comprehensive Community Action Plan by the Project Advisory Council (PAC), a council of community members and organizational representatives; 2) implementation of identified recruitment and retention strategies for AA men into the SHAPE-IT multi-faceted hypertension, stroke and prostate cancer education sessions; 3) identification of venues for increasing community awareness about stroke and prostate cancer risk factors, symptoms of disease and treatments; 4) integration of social support networks into screening and educational programs. Program evaluation data will be presented.
