Children's Futures Initiative: A public health model to create a citywide system of maternal and child health care

The Children's Futures Initiative (CF) is an interagency, public-private partnership that provides a comprehensive system of MCH services in Trenton, New Jersey. This model builds on existing services and integrates new components to strengthen the system of care and improve health outcomes for pregnant women, parents and children from birth to age three. Specific strategies and services integrated into the CF model include: 1) Prenatal screening questionnaire (includes the 4 P's Plus) conducted by OB clinical providers for risk identification, prevention education and referral to treatment/support services. 2) Central intake to streamline referral and assessment, avoid duplication, and ensure coordination/communication. 3) In-home assessment to determine woman/family needs and offer participation in CF programs and services. 4) Intensive home visitation (Nurse Family Partnership, Healthy Families, TANF Initiative for Parents) to provide long-term health education and family support for eligible families; 5) PHN home visits for women/families at any time along the continuum--pregnancy, postpartum, infancy or early childhood to give families another option. 6) Center-based activities at local Parent/Child Centers for group activities in prenatal/parenting education, breastfeeding support, infant/child growth and development, prenatal exercise, stress reduction, infant massage, early literacy and family-oriented social time. 7) Consultant services for perinatal addiction, depression, domestic violence, fatherhood support and childcare referral. 8) Perinatal outreach to promote early entry into prenatal care and community awareness about free pregnancy testing and other CF services. CF tracks data across programs to monitor success in reaching program objectives and improving health outcomes.
