Background: The national Evidence-Based (EBP) Project has been investigating the implementation of mental health EBPs in state public mental health systems for adults with serious mental illness for 5 years. The study reported is a secondary analysis of site visit key informant interview reports that described state-level activities and strategies associated with the implementation of the Project's EBPs in 8 states. Method: Qualitative data analysis methods-content analysis-and application of Greenhalgh et al's (2004) definition of innovation, were used to evaluate number and types of state-level activities and strategies associated with the Project's Pre-Implementation, Initial Implementation and Plans for Sustainability phases from initial (2002-2003) and follow-up (2004) site visits in all 8 states. Results: 106 discreet innovative implementation activities and strategies were classified into 5 types. Differrent implementation strategies were needed at different phases of implementation. The state's role was shown to be critical in the implementation of EBPs at the community level and the evolutionary model of implementation was shown to help explain the processes by which this occurs. Conclusions: This study contributes to the limited knowledge base that exists for innovations in mental health services implementation, and reports that inventory the spectrum of state-level EBP implementation activities and strategies. A variety of stakeholders can consider using the interdisciplinary theoretical bases, methods and classifications of implementation activities and strategies, to conduct further research, and help enhance the development, implementation and evaluation of EBP service delivery cultures in mental health and healthcare systems.
