Strategic design of dynamic organizations in the public health arena

In today's environment of budget cuts and emerging global health challenges, many government public health agencies are forced to redesign their organizational structure to maximize efficiency and increase public health impact. The Centers for Disease Control and Prevention (CDC) Futures Initiative has created a new strategy for the delivery of public health services that has resulted in the formation of the CDC Coordinating Center for Infectious Diseases (CCID). While the Futures Initiative formed CCID to increase the coordination between these centers, existing organizational design elements did not fully leverage cross-center collaboration nor optimize resource utilization. Through leadership retreats and working groups, CCID Leadership adopted a new four-center model. These centers were organized around vaccine preventable diseases, routes of disease transmission (food-borne/water-borne/zoonotic and sexually transmitted diseases), and preparedness and response functions to maximize functional synergies between analogous public health programs. This model also included a Strategic Business Unit and a Strategic Science Program Unit, specifically designed to increase the efficiency of CCID support resources and realize cross-center collaboration between scientific groups via horizontal integration, respectively. Design elements from this model can be applied to other public health organizations to maximize public health impact in uncertain times.
