A public health clinic and community academic center collaborated to implement an innovative model of providing goal setting and self-management support to a diabetic patient population characterized by minimal health literacy, limited English proficiency, co-morbid medical and mental health diagnoses and different cultural attitudes toward health and illness. Public health physicians, health-workers and nurses collaborated with and trained post-baccalaureate volunteer bilingual students to assist patients in jointly devising action plans to effect behavioral change. Students were trained in motivational interviewing techniques, using the Brief Negotiations model and taught to assess patients' readiness to change. Students then worked in the clinic with patients, directly after the provider visit, to help with problem solving and setting self-management goals. Students also made follow-up phone calls to determine if goals were achieved and trouble shoot with patients on unattained goals. Patient self-efficacy and improvement in quality indicators, such as A1C, LDL and blood pressure levels were monitored. This presentation will discuss the process used to implement this service model and will provide information on how interested agencies can replicate this model.
