Objective: The Off (Officially Free From) Nicotine Teen Tobacco Cessation Program is a cognitive behavioral cessation program consisting of 4 ninety-minute sessions presented once a week and one booster session one month following program completion. Program features include: the Teen Smokers CAGE Comprehensive Tobacco Assessment, use of exhaled carbon monoxide monitors at each session, and a participant workbook utilizing evidence based programming and innovations developed for this program. The Off Nicotine program is offered to adolescents in Cuyahoga County, primarily in school-based settings. Methods: Program evaluation is performed using pre- and post-tests collected at cessation sites across the county. The pre-test (Teen Smoker's CAGE) examines tobacco use patterns, self-image as a smoker, and affect toward cessation. The post-test data includes successful cessation and reduced smoking at program conclusion, susceptibility to relapse, relapse triggers, participant satisfaction with individual program components and satisfaction with the overall program. Exhaled carbon monoxide levels are used to verify self-report at program completion. Follow up is conducted at 30 days, 6 months and 1 year to further evaluate successful cessation and reduced smoking rates. Results: Preliminary results indicate high rates of participant retention with 87.5% attending at least 3 of 4 sessions. Quit rate at program completion was 75%. Cut down plus quit rate was 87.5%. Additional follow-up data will become available. Conclusion: The Off Nicotine program is emerging as an innovative, flexible and effective teen smoking cessation program.

Learning Objectives: At the conclusion of the session, the participant (learner) in this session will be able to
