Waist circumference, waist-hip ratio and body mass index : Their correlation with abnormal liver function risk factors in Korean adults population

The aim of this paper is to compare waist circumference(WC), waist-hip ratio(WHR), and body mass index(BMI) as indices of obesity and to assess their role in abnormal liver function. We analyzed adults data ages 20 yr and older(N=5.734) from Korea Health and Nutrition Survey conducted in 2001. We defined the abnormal liver function if SGOT is over 40IU/L or SGPT is over 40IU/L. In order to determine obesity as a significant factor of abnormal liver function, we calculated and compared the well known obesity indices : WC, WHR, and BMI. Also, we analyzed their correlation with the abnormal liver function with odds ratios. The study subjects included were 2,442 men and 3,292 women aged 20 years and over. The mean age of the total subjects was 45.10(±15.55). Overall abnormality rate of liver function test was 7.8%. The obesity prevalence data by WC, WHR, and BMI were 33.24, 42.20 29.63% respectively. The correlation coefficients of SGPT with the obesity indices were 0.397, 0.354, 0.332. The kappa values among obese subjects were 0.2504(BMI with WC), 0.2515(WC with WHR), 0.5916(BMI with WC). The odds ratios of abnormal liver function in obese subjects compared with non-obese were 1.92(95% CI; 1.58, 2.34), 2.19(1.80, 2.67), 2.91(2.39, 3.54) by WC, WHR, and BMI indices. Even though the indices were good measures for estimating both prevalence of obesity and abnormal liver function risk factors, WC is better measurement for obesity and to identify those with abnormal liver function with the narrowest CI's.
