Chronic Disease Model and illicit drugs

This session will report U.S. and Canadian data on illicit drug trends, a scan of current illicit drug use evidence-based intervention (treatment) initiatives which demonstrate the documented benefits of linking epidemiology with practice, and reflect a continuum of care model in keeping with other chronic disease models drawing from newly tested evidence-based models. Recommendations will be offered linking illicit drug use data and tie them to risk factors for chronic disease interventions as articulated by the MacColl Institutes as the community, the health system, self-management support, the design of the delivery system, decision support, and clinical information systems.
