Quality of antenatal care before and after the implementation of prevention of mother-to-child transmission of HIV in Côte d'Ivoire

Background: This study aimed at assessing the quality of antenatal care before and after the implementation of a prevention of mother-to-child transmission of HIV (PMTCT) programme in Côte d'Ivoire. Methods: A baseline study was conducted in 2002/3 in five urban health facilities in San Pedro and Abidjan. PMTCT was implemented in all five health facilities during the first semester of 2004. The “after” study was conducted in 2005 in the same facilities. Each phase of the study consisted of a standard description of the facility, observation of antenatal consultations and exit-interviews of women (n= 606 before and n= 591 after). Results: Inter-personal relationship improved in all health facilities. Clinical examination at first visit did not significantly change. However, checking fetal position and fetal heart rate at subsequent visit during second or third trimester increased from 75% to 88%(p<0.001)and from 80% to 88% (p=0.01), respectively. HIV testing, never mentioned at baseline study, was proposed to a majority of women attending the first antenatal visit after PMTCT implementation. Some indicators, remained poor, for instance hands washing (before or after the consultation) was done in 11% of cases after the implementation of the programme versus 3% before. Conclusions: Our results show that PMTCT implementation in these five health facilities in Côte d'Ivoire had some impact on the quality of antenatal care.
