Insights into the provision of oral hygiene care in nursing homes

The provision of oral hygiene care for residents of nursing homes is one of the most challenging activities for carers, especially with the many older residents with dementia and related disruptive behaviors. Longitudinal oral epidemiological data from US and international nursing homes studies have substantiated the high levels of plaque accumulation and oral diseases observed clinically in residents. This presentation will describe the disruptive behaviors observed during oral hygiene care with residents, including those with dementia and other neurological disorders. Information from several recent studies by Coleman and Chalmers that observed oral hygiene care in nursing homes will highlight procedural, organizational and carer influences on the provision of oral hygiene care. A model will illustrate how these and other influences are intertwined in a complex cycle that results in inadequate oral hygiene care provision for residents in many nursing homes (Chalmers, Cody, and Pyfferoen, 2006). Several oral health promotion initiatives for medical and nursing staff as well as Certified Nursing Assistants (CNAs) will be discussed; these include a national physician oral health toolkit (Chalmers, 2006, Commonwealth Department of Health and Aging, Australia), a story-based interactive web training course for CNAs (Iowa Geriatric Education Center), and a best-practice model for improving older adults' oral health for those that are homebound (Iowa's Seniors Smiles) and residing in nursing homes (Iowa and Australia).
