Learning Objectives: 1. Distinguish between clinical preventive services and self-management practices in diabetes care; 2. Describe the pay-for-performance payment incentive program designed to improve quality of care; 3. Recognize that only moderately high percentage of patients with diabetes and advanced chronic kidney disease receive ACE/ARB prescription and older age is associated with a lower prescription rate; 4. Recognize risk factors associated with bariatric surgery; 5. Describe quantitative and qualitative data from the first year of the New York City Dept. of Health and Mental Hygiene’s innovative Preventive Services Advisor (PSA) program, an initiative to assist primary care practices in New York City move toward quality improvement in diabetes care. | |||
