An instrument, the Handbook for Managing Your Health Needs: A guide for people with lower limb mobility impairments and mobility limitations for reducing health problems, improving fitness and tracking items needed for everyday living was developed to assist people with mobility impairments in tracking and managing their own health. The handbook was developed through a program sponsored by the Missouri Foundation for Health in which participants receive a 2 hour orientation and training on the handbook and have an option to participate in a free 12-week exercise program. Participants receive training on tracking their health history, health risks and secondary conditions and are surveyed pre and post participation in the program on their health status, secondary conditions and participation in activities. The poster will describe the contents of the handbook and qualitative data gathered through surveys and focus groups about participant use of the handbook, health status and secondary conditions.

1. Identify secondary conditions that may impact the participation of people with mobility impairments. 2. Describe one method of tracking secondary conditions and health factors that may impact the participation of people with mobility impairments. 3. List benefits of developing a health handbook and training people with mobility impairments to use a health handbook.
