180589 Community Health Worker (CHW) perspectives from California's Corzaon de la Familia Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN) ProjectTuesday, October 28, 2008: 2:50 PM
Corazon del la Familia, California's WISEWOMAN Project, is a bilingual, CHW-led, clinic–based cardiovascular risk reduction project for Latino women 40-64 years of age. Women must be enrolled in the National Breast and Cervical Cancer Detection Program to be enrolled in WISEWOMAN. Clients are enrolled into the project if they are of Latin decent, had high blood pressure and or cholesterol or are on medication to control their blood pressure and cholesterol levels. CHWs administer nutrition, physical activity, and stage of readiness to change assessments. CHWs use these assessments and the Vida Saludable, Corazon Contento (VSCC) curriculum to negotiate and guide women to set healthier eating and physical activity goals. In this conference session the California WISEWOMAN Consultant and three CHWs from different participating clinics will present on the barriers, successes and opportunities in: 1) using the VSCC to guide clients toward behavior change; 2) making choices for education materials and incentives to enhance behavior change 3) goal setting, 4) effectiveness of one-on-one counseling and group sessions.
