The rise of chronic diseases manifest itself in communities, countries and globally. Among chronic diseases, heart disease and stroke are the leading causes of morbidity and mortality both in the US and the world. Interdisciplinary efforts are vital to meeting the challenges of the prevention and management of heart disease and stroke given the complexities of risk, system and behavior. The state of collaborations, opportunities and barriers, how to join in the Forum or other collaborations, role of The APHA Affiliates, and research oriented members, related to further participating in the network will be discussed.
Session Objectives: The participant will be able to identify the importance of national and global alliances in reducing the burden of heart disease and stroke through the work of the Medicine and Public Health Initiative and the National Forum for Heart Disease and Stroke Prevention; local, state, international and voluntary efforts for prevention and management of heart disease and stroke; and opportunities for APHA member engagement.
