Objective

Hospitals have been increasing their efficiency of medical care and promoting early hospital discharges. Under these conditions, it is important for nurses to provide appropriate nursing services to address concerns of patients and their families. An important issue is training of nurses to have the ability to continue nursing even if they move to the place of any kind of medical treatment in the community. The purpose of this study was to develop an education program and System for Acute hospital nurses who will have the ability to continue nursing care.

Methods

We used action research. The participants were the representative of three organizations in the A prefecture. They were a Managing Director of Nursing Association, a Chairperson of Association of Home Health Nursing agency and A Nursing Director of National University Hospital. They planned an education program for nurses to train the ability to continue nursing together. They carried out the program oneself. They confirmed the learning effect of the learner with reflection sheet after program. Furthermore, they talked about the significance that three organizations collaborated each other. An Education program was 5times sessions including lecture,group work and self-exercise during 6th months. The participants and learners filled out reflection sheets after each programs.

Results

The results of the reflection sheets indicated that the nurses attending the programs were becoming clearer on their own issues described below as they continued. The issues were “providing nursing care for families of patients who are to be discharged soon,” ” managing nursing care for the patient and their family hope ” “understanding a social insurance system in the community health care,” “utilizing a system supporting discharge during hospitalization,” and “understanding activities of home care nurses.”

Conclusion

Our program helped clarify the nursing issues of nurses working in hospitals. These issues are necessary in the discharge support. In the future, we will be measuring the effectiveness of the training program. That is, a follow-up study will be performed to examine what type of discharge support is being performed within the hospital by the nurses who have taken this program.
