207507 Sexual health disparities in a national sample of U.S. college students

One overarching goal outlined in Healthy People 2010 and Healthy Campus 2010 is to eliminate health disparities among different segments of the U.S. population. Little is known, however, about the sexual health disparities among college students. We examined such disparities between blacks and whites in a national sample of U.S. college students, utilizing data from 44165 non-married undergraduates (aged 18-24 years) responding to the Spring 2007 American College Health Association-National College Health Assessment. Chi-square cross-tabulations and independent samples t-tests were conducted to assess differences between black and white students. When reporting on lifetime sexual behavior, a significantly greater proportion of white students reported oral sex (72%) than their black peers (58%), and a greater proportion of white women reported anal sex (21%) compared with black women (17%). Black students were significantly more likely to use a condom for last oral, anal, and vaginal sex, but also had more sex partners in the past year (Mean[blacks]=1.72 [SD=3.49] and Mean[whites]=1.40 [SD=2.36]; p<.001). Proportionately, almost twice as many black men (19%) reported having ≥4 sex partners than white men (11%). Almost twice as many blacks reported HIV testing (42.6% vs. 23.6%), and twice as many reported having had a sexually transmitted infection in the last school year (7.8% vs. 3.8%). Lastly, a greater percentage of black students (6.6%), compared with white students (1.7%), reported an unintended pregnancy within the last school year. Thus, serious sexual health disparities exist among U.S. college students. Implications for college health and future directions for research are presented.
