The clinician is able to use a pro active fax referral form to enroll patients in counseling. The partnership between MQC and the clinician offers a comfortable forum for assessment of the patients smoking status and advice about changing their consumption. The fax provides clinicians with an easy to use referral source. MQC offers each client a pregnancy/post partum individual treatment plan using behavior modification. We provide one on one cessation counseling as well as telephone support. We meet our clients in a location that they can get to easily. WIC sites, prenatal clinics, as well as other community sites. Over the past 7 years MQC has found the best approach to reach women during preconception, pregnancy and those parenting young children is to partner with the medical community who treats and
