The Southern NJ Perinatal Cooperative (SNJPC), a regional maternal and child health consortium, assumed responsibility for Early Intervention Service Coordination in Camden County in 2005. Over 700 cases were initially transferred from the county health department and services were efficiently managed by SNJPC over a four year period. Building upon this accomplishment, SNJPC applied for and was awarded funding to expand EI Service Coordination to multiple, contiguous counties in the region. In order to contain costs, SNJPC Service Coordinators are given the appropriate equipment, guidelines and supervision to work from their homes. The key to the program's success is a secure web-based Service Coordination Information Management System (SCIMS), which can be accessed by Service Coordinators and their Supervisor. The presenter will describe the Telcommuting model, guidelines and data system in detail so that the attendee will be able to replicate this model in their practice area.
